x4mtDr3gNHw&NR=1 would actually be missing a character. It should be x4mtDr3gNHw with one more character before it. Take, for example, this:
http://www.youtube.com/watch?v=xxRXPLRiOoc&feature=related&whatever=whatever
When you see question marks and ampersands in an URL, the "?" starts the first variable, and "&" starts each variable after that. Everything before the "?" is the address of the page itself.
